Prerequisite Documents Guidlines:


NOTE: Form will not be saved until all mandatory fields are entered, and scanned images/pdf of Photo, PAN, Aadhar, and atleast one qualification is uploaded.

Instructions for upload of Photo / PAN / Aadhar Card / Relieving Letter / Certificates:
1. Photo should be colour photograph without border and clearly showing the face (Passport Size).
    Photograph must be a recently clicked or should not be more than 6 months old.
2. The minimum dimension of images must be approx 200px*200px(width*height) and not more than 2 MB in size.
    Format may be(jpeg, jpg or png).
3. Aadhar Card, Academic certificate's and Relieving Letter should be in PDF format, with no more than 2 pages, with clearly readable content.

Aadhar Card Detail:
Digital Aadhar Card without password lock or scanned image of Aadhar card in PDF format is to be uploaded.
Please do not upload smart-card size copy of the aadhar card. Aadhar card copy should contain "W/O" or "S/O" or "D/O" line.

Steps to Download the latest aadhar card and remove password lock
  1. Download PDF from myaadhaar.uidai.gov.in
  2. Open the PDF in any browser or in Adobe Reader and enter the password, eg:"PASS2001" - 1st four letter of your name and birth year.
  3. Click on Print or enter "Ctrl + P" and Select Destination as "Save As PDF" / "Microsoft Print to PDF"
  4. Click "SAVE" button, a new PDF without password will be saved in local directory.

Steps to make PDF document from image:
A.Microsoft Office Word:
  1. Open MS-Word, Copy and Paste scanned images in the document, an image per page.
  2. Goto File -> Save as -> Select file type as PDF and save in local directory.
B.Google Docs:
  1. Open Google Docs -> Select a new documnet. Copy and Paste scanned images in the document, an image per page.
  2. Goto File -> Download -> PDF Document -> Save in local dirctory.
C. On Libre Office:
  1. Open Libre Office Writer/Word, Copy and Paste scanned images in the document, an image per page.
  2. Goto File -> Export as -> Export as PDF option and click, it will open pop-up to save file as PDF in local directory.
